Health Homes Care Management Internship

The Home Health Care Management Internship is a unique opportunity to assist families through connection to resources. The intern will learn from the current Care Managers to understand the programs available through the health care system.  The intern will learn how to enroll families into such programs.  The Health Homes program assists families with access issues, developing relationships with service providers, and tracking interventions and outcomes. The intern will learn how to engage families, and assist with interventions to help prevent and resolve crisis situations. 

Community Based Services
 Graduate Intern

Interns will be exposed to the Outreach Center, shelter, and other programs that provide support to families and youth in the community. Interns will have the opportunity to interact with clients individually and/or assist  with case management. Interns will focus on three clients  and keep updated client files and progress notes, facilitate groups and plan activities for these youth, attend staff meetings and participate in outreach activities in the community.

Community Based Services Undergraduate Intern
Interns will be also be exposed to multiple counseling and supportive settings. Interns at this level will assist with two clients specifically but will also learn about the process of keeping appropriate progress notes,  how to facilitate meetings, help plan activities for youth, attend staff meetings and participate in Outreach Day.

Because this is an unpaid internship, interns are expected to earn academic credit for this experience. Please consult with your faculty advisor.

Qualifications:

  • First or second year Master’s level student
  • Experience with and desire to work with youth and/or teens
  • Flexibility and adaptive nature
  • Must receive credit for participation in the internship

Community Based Services Undergraduate Intern
