Bilingual Health Home Care Manager

The Care Manager is the key point of contact in managing and coordinating medical, educational, and behavioral health services for youth and families by addressing access issues, building relationships with providers, and tracking interventions and outcomes. The Care Manager works with each family to ensure all providers are working together with the youth's best interest in mind to identify and leverage their strengths, empowering them to succeed at home and in the community. The Care Manager will conduct comprehensive assessments in order to obtain the most accurate information to build off of the identified needs and risk factors.

Education/Experience:

 

Health Home Care Managers that serve High acuity enrollees must have the following minimum education and experience:

A Bachelors of Arts or Science with 2 years of relevant experience working with youth and families in child welfare, developmental disabilities, mental health, healthcare, and/or other systems, as well as those receiving preventive services,

or

A Master’s Degree with one year of relevant experience.

Licensure/Certification:

 

Valid driver’s license and a registered operating car to be used for daily transportation for work related purposes

 

BilingualSpanish with bonus

Pay Rate: $38,000-$45,000 peryear depending on education and experience.
