Counselor (Home and Community Based Services)

The HCBS Counselor (Home and Community Based Services Counselor) works with youth and their families to support and achieve optimal functioning in their home, school and community. Specific responsibilities include but are not limited to: identifying predictors of violence in the home and community, recognizing and addressing signs of escalation, maintaining familiarity with the child’s Safety Alerts Plan (SAP), supervising child’s activities, communicating clearly with the child, offering feedback to the child and family, identifying activities that support the targeted developmental tasks, strengths, and/or talents, utilizing community resources, maintaining on-going communication with OLP and or Care Managers, informing all team members of child’s progress, concerns and changes in child’s behavior, completing HCBS Progress Notes filling in all required billing elements, ensures that billed hours of contact match time sheet, adheres to NYS and agency requirements including: HIPPA, ethical guidelines, confidentiality, and worker safety protocols, and other responsibilities as assigned.
What you bring to Green Chimneys:
Bachelor’s level degree with a minimum of two years of applicable experience in children’s mental health, addiction, and /or foster care/child welfare/juvenile justice.  Have experience working with children with special needs (preference given to those with experience). Be supervised by an individual who meets the criteria for a "qualified mental health staff person" found in 14 NYCRR 594 or 14 NYCRR 595. Valid driver’s license and a registered operating car to be used for daily transportation for work related purposes. Ability to complete OMH’s 3-day Skill Building Curriculum training which includes training on the Individualized Care Model.

This is a full time and benefit eligible position.
The rate for this role is $43,290 per year.
