Employment

The Care Manager acts as the key contact for managing and coordinating medical, educational, and behavioral health services for young people and their families. They work on resolving access issues, building strong connections with service providers, tracking progress, and identifying any barriers to care. By working closely with each family and care team, the Care Manager ensures that all providers are aligned in their dedication to the youth's well-being, helping to identify and leverage their strengths, thus empowering them to succeed both at home and in the community.

Education/Experience:

• Minimum of an Associate’s Degree with two years of experience is required
• Bachelor's Degree with two years of experience is preferred. Candidates who can demonstrate they are working toward a Bachelor’s degree with a graduation date within one year can be considered. 
• Relevant expertise and experience in serving children and families in child welfare, developmental disabilities, mental health, healthcare, and/or other systems, as well as those receiving preventive services.
• Demonstrated understanding of the needs of high acuity children and their families as evidenced by additional years of experience, education, and training.
• Bilingual is a plus.

Licensure/Certification:

• Valid driver’s license and a registered operating car to be used for daily transportation for work-related purposes

• This is a full-time and benefit-eligible role.

Pay Rate: $38,000-$45,000 per year, depending on education and experience.
